Our primary objective is to assist communities in enhancing and broadening treatment and recovery support services for individuals, including youth and families, experiencing homelessness, particularly those with substance use disorders or co-occurring mental and substance use disorders.

Outreach & Engagement

We reach out to individuals in need and actively engage them in our supportive programs.

Benefits Enrollment

We assist in accessing essential benefits such as Health Insurance, SSI/SSDI, and other mainstream benefits to ensure holistic care and support.

Case Management & Recovery Support

Our dedicated case managers offer personalized support to navigate the complexities of recovery and rebuilding life.

Screening & Assessment

We provide assessments for both Mental Health and Substance Use Disorders to identify individual needs and tailor our services accordingly.

Direct Treatment

Our team offers evidence-based treatment for Co-Occurring Disorders and/or Substance Abuse related health issues.

Recovery Support

We connect individuals with a network of resources to empower them on their path to recovery.

Permanent Housing

We help individuals secure stable housing, providing a crucial foundation for their recovery journey.

To self-refer for support services or to refer a person in your care, simply click the link below to complete a short survey. Within 24-48 hours, an advocate from our team will reach out to provide a full assessment for support services.
