FLACRA’s Care Management program assists individuals with chronic medical, mental health, or substance use conditions through care coordination and community-based services. Care Managers ensure providers collaborate, support wellness, and promote self-sufficiency while connecting clients to housing, food, childcare, and transportation
Care Management (HHUNY/CHHUNY)
Health Home services are contracted through Health Homes of Upstate New York Network (HHUNY), a care management network serving Medicaid members with complex physical, behavioral, or social needs. A dedicated Care Manager coordinates comprehensive, goal-centered care.

Children’s Health Home of Upstate New York (CHHUNY) focuses on children and families, helping navigate healthcare and social systems, connect to community resources, and tailor services to individual needs for eligible adults, children, and young adults across multiple Upstate New York counties.

HCBS/CORE
Home and Community-Based Services (HCBS) and Community Oriented Recovery and Empowerment (CORE) provide Health and Recovery Plan (HARP)-eligible Medicaid beneficiaries peer support at home and in the community. Services support education, employment, daily living skills, rehabilitation, therapy, family engagement, and empowerment for individuals with intellectual, developmental, mental health, or substance use challenges, promoting independence, recovery, and resilience through person-centered, trauma-informed care.
