Adult, Children and Youth Care Management Services
FLACRA’s Care Management program assists individuals experiencing chronic medical, mental health and/or substance use disorder conditions with care coordination and related community-based services. Our Care Managers assist clients in coordinating mental health, substance use, medical and other appointments as needed. They ensure all providers are working together to collaborate and share treatment information to better support the client’s individual wellness and promote self-sufficiency.
Health Home services are contracted through Health Home of Upstate New York Network (HHUNY). Care Management services are available to residents of Wayne, Ontario, Seneca, Yates, Monroe, Schuyler, Steuben and Counties.
Children’s Health Homes of Upstate New York (CHHUNY) focuses on the needs of children. Care Managers help navigate finding the right resources for those in need
Qualifications for FLACRA Adult Care Management:

Potential clients must have Medicaid
-
AND reside in Monroe, Ontario, Schuyler, Seneca, Steuben, Wayne, or Yates Counties

AND have been diagnosed with a serious mental illness
-
AND diagnosed with HIV/AIDS
-
OR have two or more of the following conditions: a Substance Use Disorder, Mental Health Condition, Asthma, Diabetes, Heart Disease, a BMI over 25 or another chronic condition.
Qualifications for Child and Youth Care Management:

Potential clients must have Medicaid
-
AND reside in Monroe, Ontario, Schuyler, Seneca, Steuben, Wayne, or Yates Counties

AND have two chronic conditions

OR diagnosed with HIV/AIDS

OR complex trauma
-
OR serious emotional disturbance
-
AND have significant behavioral, medical or social risk factors which can be addressed through care management
