Care Management

FLACRA’s Care Management program assists individuals with chronic medical, mental health, or substance use conditions through care coordination and community-based services. Care Managers ensure providers collaborate, support wellness, and promote self-sufficiency while connecting clients to housing, food, childcare, and transportation. Services are available to residents across multiple Upstate New York counties. Health Homes of Upstate New York (HHUNY) supports Medicaid adults with complex needs, while Children’s Health Home of Upstate New York (CHHUNY) provides family-centered care management for youth.

Residential Treatment

FLACRA's residential services provide individuals with substance use disorders a safe living environment with support and services available to encourage a successful recovery. Our stable, 24-hour supervised housing treatment program offers clients individualized education and guidance with activities of daily living, counseling and vocational education services.
